Insurance

Our office participates with Medicare. This means that Medicare will pay 80% of all covered services. You are responsible for your deductible, any non-covered services, and the 20% that Medicare does not pay. Payment is due at the time of service. Please note that refractions are not covered by Medicare.

In addition to Medicare, our office participates with Blue Cross and Blue Shield of Michigan, Blue Cross and Blue Shield of Michigan Vision, and Preferred Choices. We are Medicaid (Michigan only) and Children Special Health Care providers. If you are not covered by the above carriers, your insurance carrier will reimburse you directly for all covered services.

When non-surgical services are rendered, simply fill out the health insurance claim provided by your carrier and attach our paid receipt. It will serve as the “attending Physician’s statement” and you should need no other paperwork from our office. We require payment at time of service. Of course, if you encounter any difficulties, we will be happy to assist you in filing your own claim for primary and secondary insurance carriers.

As a courtesy to our patients, we will process claim forms for surgical procedures. We request that you provide us with your complete insurance information. Non-covered fees, deductible amounts, or claims not paid within reasonable amount of time become your responsibility. Upon your request our Medical Billing Assistant will attempt to contact your carrier prior to your surgery and will advise you, when necessary, of your responsibility. Your portion of the charges is due prior to surgery.

For your convenience, we accept MasterCard, Visa and Discover, in addition to cash and your personal check.
